چکیده
Recurrent Respiratory Papillomatosis (also known as Laryngeal Papillomatosis) is a benign and sporadic tumor that primarily affects children and is caused by the papillomavirus. The estimated prevalence of this condition is approximately 4 cases per 100,000 children and 2 cases per 100,000 adults. Human papillomavirus types 6 and 11, which are commonly associated with genital warts, are the predominant strains implicated in this disease. The most common symptoms include airway obstruction, voice disturbances, and difficulty speaking. These lesions are typically integrated and are rarely observed individually; they sometimes progress toward malignancy. A total of thirty-one laryngeal samples from patients with a positive pathological response to recurrent respiratory papillomatosis RRP were collected from the hospital's ear, nose, and throat department. Each sample was preserved in formaldehyde and embedded in paraffin blocks. Accompanying the samples was a form containing detailed patient information. After analyzing the presence of the β-globulin gene in the DNA of the samples, specific primers (MY09/11 and GP5+/6+) were employed to detect the presence of human papillomavirus (HPV). Among the 31 samples, 29 were found to contain the HPV genome, with HPV-6 identified in 13 samples and HPV-11 in 16 samples. The phylogenetic tree of isolated HPV viruses was subsequently plotted. Statistical analyses revealed no significant difference in the incidence of HPV viruses between men and women, nor in the incidence of RRP. However, a significant correlation was identified between residing in suburban areas, low income, and welfare levels, and the incidence of RRP. Additionally, the research indicated that RRP lesions predominantly affect pediatric patients, with only a small percentage of adults being affected. Further extensive studies are necessary to elucidate the main risk factors associated with RRP patients.

تعیین ژنوتیپ ویروس های پاپیلومای انسانی در بیماران مبتلا به پاپیلوماتوز تنفسی راجعه در بیمارستان فیروزگر، تهران، ایران

پاپیلوماتوز تنفسی عود کننده Recurrent Respiratory Papillomatosis) ( یا پاپیلوماتوز حلقی (Laryngeal Papillomatosis) یک تومور خوش‌خیم و بسیار نادر در کودکان در اثر پاپیلوماویروسها است که میزان شیوع آن در حدود چهار مورد در صد هزار کودک و دو مورد در صد هزار بزرگسال برآورد شده است. تیپ‌های 6 و 11 پاپیلوماویروسهای انسانی که تیپ‌های معمول نواحی ژنیتال هستند ویروسهای غالب در این بیماری هستند. این تومور معمولا در محل تلاقی ناحیه قدامی سومین گره صوتی آغاز شده و ممکن است کل حنجره را نیز درگیر کند. گرفتگی راههای هوایی و اختلال در صدا و صحبت کردن از متداول‌ترین نشانه‌های این بیماری است. این ضایعات معمولا به صورت مجتمع بوده و به ندرت به صورت منفرد دیده می‌شوند و گاهی اوقات ضایعه RRP به سوی بدخیمی پیش می‌رود. 31 نمونه‌ لارنکس بیماران با جواب پاتولوژی مثبت RRP شامل 17 مرد و 14 زن که به صورت فیکس شده با فرمآلدهید و بلوکهای پارافینه بودند از بخش گوش، حلق و بینی بیمارستان به همراه اطلاعات بیماران شامل سن، جنس، علائم اولیه بیماری، سن اولین جراحی، تعداد جراحیها و فواصل بین آنها، نواحی درگیر، سابقه تراکئوستومی، دریافت اینترفرون و بهبودی نیز مورد بررسی قرار گرفتند. پس از بررسی حضور ژن Globolin-β در DNA نمونه‌ها، برای تشخیص حضور HPV از پرایمرهای اختصاصی MY09/11 و سپس GP5+/6+ استفاده شد. 29 مورد از 31 نمونه‌ حاوی ژنوم HPV بودند و نمونه‌های HPV مثبت با روش تعیین توالی و با استفاده از سایت NCBI ژنوتایپینگ شده و مشخص شد که 13 نمونه حاوی HPV-6 و 16 نمونه حاوی HPV-11 بودند. سپس درخت فیلوژنیک ویروسهای HPV جداسازی شده برای ثبت در بانک ژنتیک ویروسهای HPV رسم شد. آزمونهای آماری در این مطالعه نشان دادند که تفاوت معناداری بین میزان ابتلای مردان و زنان به ویروسهای تایپ HPV و ایجاد ضایعه RRP در آنها وجود ندارد ولی ارتباط معنی‌داری بین زندگی در مناطق حاشیه شهر و با سطح درآمدی و رفاهی پایین و ابتلا به RRP وجود داشت. همچنین مشخص شد که ضایعات RRP عمدتا بیماری اطفال بوده و درصد کمی از مبتلایان در این تحقیق بزرگسال بودند.
